 Spring/Summer Camp Required Forms
Find the required forms for your child's camp below. The following forms are included in each packet:
- Parent Letter
- Release of Camper
- Medical Form
- Administration of Medication
- Acknowledgement of Risk
Please Read: Each child must have these completed forms in order to participate in the camp program. Forms cannot be submitted through email or online. You cannot save data typed into the form.
If your child requires any prescription or over-the-counter medication during the day, you must have the Administration of Medication form(s) completed by your doctor. The medication must be in the original container with clear instructions on both schedule and dosage.
